AUZ Care's Plan Management Service Agreement applies to Registration Group 0127 – Plan Management and sets out how AUZ Care provides Plan Management services to participants.
About this agreement
Plan Management services are limited to financial administration of NDIS plan funds, including invoice processing, claims submission, provider payments, budget tracking and financial reporting.
This agreement does not cover support coordination, direct service delivery or monitoring the quality or outcomes of supports delivered by service providers.

Participant Rights
- Exercise choice and control over service providers
- Access financial records, invoices and statements
- Raise complaints or concerns without fear of retaliation
- Change Plan Managers in accordance with NDIS requirements
Participant Responsibilities
- Notify AUZ Care if circumstances or contact details change
- Inform AUZ Care if service providers or Plan Manager arrangements change
- Review and approve invoices where required

Services Provided
What AUZ Care provides
- Processing and payment of invoices from registered and unregistered providers
- Submission of NDIS claims in accordance with NDIA requirements
- Monitoring and tracking of participant plan budgets
- Monthly financial statements and expenditure reports
- Responding to participant enquiries relating to plan funding and expenditure
Systems & safeguards
Services are delivered using structured financial workflows and approved systems, including Brevity Plan Management software, to support invoice validation, price-limit compliance, secure record keeping, audit trails and reporting.
Privacy and complaints
AUZ Care collects, uses, stores and discloses personal and financial information in accordance with the Privacy Act 1988 (Cth) and AUZ Care's Privacy and Record Keeping Policy. Complaints and incidents are managed in accordance with relevant AUZ Care policies.
Ending the agreement
Either party may end the agreement by providing fourteen (14) days' written notice. With participant consent or lawful authority, AUZ Care will take reasonable steps to support an orderly transition of records and relevant information.
